We are   in -network provider for most of the health plans under the insurance companies listed below. If you are not sure if your plan is included, please fax or e-mail a copy of the insurance card, along with a full name and date of birth of the patient, and we will check the information for you.
Please note that Mental Health Benefits often have their own set of rules and requirements, different than your other medical benefits (for more please go to "Patient Forms").

Fee-for-service

I also provide fee-for-service appointments. Professional fee schedule is available upon request.  Fee-for-service offers most flexibility in treatment choices, as we do not have to seek the approval of insurance company.

If I am considered an out-of-network provider for your insurance, I will fill out forms and provide you with whatever assistance I can in helping you receive the benefits to which you are entitled; however, you (not your insurance company) are responsible for full payment of my fees. It is very important that you find out exactly what mental health services your insurance policy covers.

You should also be aware that most insurance companies require you to authorize me to provide them with a clinical diagnosis. Sometimes I have to provide additional clinical information such as treatment plans or summaries, or copies of the entire record (in rare cases). This information will become part of the insurance company files and will probably be stored in a computer. Though all insurance companies claim to keep such information confidential, I have no control over what they do with it once it is in their hands. In some cases, they may share the information with a national medical information databank. I will provide you with a copy of any report I submit, if you request it.

Our contract with Humana terminates on 01/31/2012

Humana has just cut our rates by 50%. We have tried to re-negotiate, but the answer was that they have enough doctors in the network and simply they don’t need to do it.

There are two options that are available for you:

1)Continue to see Dr. Gluszak on self-paid terms, and then try to get reimbursed by Humana for the        part of the cost. How much or if any amount will be reimbursed depends on your plan’s out-of-                      network benefits.

Our rate for 15-20 minutes follow up is $100, for 50 minutes therapy and medication management $150.

We would fill the reimbursement form with all the information and mail it to you. You will only have to date it, sign and mail it to Humana. We recommend mailing it via certified, trackable mail. Up to six visits can be filed on each reimbursement form. Humana Claim address:

2)Find another provider

You may also contact your Humana plan about re-considering its position regarding Dr. Gluszak’s contract. This would be very much appreciated.
